THE COACH PHIL FUND-HELPING CHILDREN PLAY HOCKEY
THE COACH PHIL FUND IS A FINANCIAL ASSISTANCE PROGRAM DEDICATED
TO THE MEMORY OF COACH PHIL ANDERSON. HE BELIEVED THAT NO CHILD SHOULD NOT BE ABLE TO PLAY HOCKEY BECAUSE OF FINANCIAL
SITUATION. THE FUND IS AVAILABLE TO GIRLS AND BOYS AGES 4 AND UP. 1 HR OF VOLUNTEERISM TO THEIR RECIPIENTS HOME
ASSOCIATION IS REQUIRED FOR EACH $20 OF FINANCIAL ASSISTANCE. IF WE CAN HELP YOUR CHILD PLAY HOCKEY, PLEASE DOWNLOAD

Sponsorship applications and ad sales forms are available
from your team manager. Please remember that you will be credited $50 for each sponsor tag sold up to the cost of your
player's total fees. Each player (Girl's teams) will be required to sell $200 in tournament ads to defray the cost
of no gate fees this year at the Sarah.
Adopt a Lady Lion Program
It is the philosophy of the
Erie Lady Lions Girls Hokey Association to encourage the participants to raise money to help support their hockey. Through
the Adopt A Lady Lion Program, our girls can earn money to subsidize the ever rising cost of playing hockey.
- The Adopt a Lady Lion form can be downloaded from our website.
- Please complete two forms for each sponsorship.
- The Adoptee keeps on copy and the other goes to our Treasurer.
- Please make sure that all forms are signed and a check is attached.
- Sponsorships will not be valid nor credits issued to players until payment is made and deposited.
- The completed form must be sent to: Erie Lady Lions Girls Hockey Association, ATTN: Treasurer, P.O. Box

Levels of Adoption:
- $150 Bronze(One Home or Away Jersey)
- $250 Silver (Same as Bronze with a website banner & link)
- $500 Lions Line (Five Home or Away Jerseys)
- $2250 Gold (Home Team or Away Team Jerseys)
If your daughter did not get
a sponsor, the ELLGHA reserves the right to put a sponsor on her jersey.
